The Loudpipe package contains paperwork to ensure that proper recourse and security for all parties are assured. It is imperative that the paper work is completed proficiently and submitted directly to the Operations Office for approval.

Please initial on the left hand side, place all the paperwork including this CHECK LIST into an envelope, and address it to the Operations Office of LoudPipe Communications Ltd.

Errors, omissions, and illegible writing may cause a delay in commission or default in commission payout.
